RC-135CrewChief
This was at RAF Lakenheath around 1999. The 48th FW lost 5 aircraft that year.

They were doing an engine test in full afterburner when the hold-back chain failed, punching the jet thru the hangar door. Became a Class A mishap (>1m $ damage)

In fact, on fighters, MOST engine testing is done indoors. Way easier to control fod, Hold downs, and anchor chains more easily available, and the neighbors don't complain about the noise when done in the hush house.

This is what happens when the hold back cable fail INSIDE what is called a "SOUND SUCKER". They can run an airplane in full burner and you can't even hear it outside of the building. Kind of cool.

The aircraft holdback didn't fail....they installed it incorrectly on the tailhook. And yes, they do test engines indoors. I worked in a hush house at Eglin AFB for 6 years(we didn't drive a plane thru the doors thought :)

Only the US Airforce would do engine run in stage 3 afterburners in side a hangar. LOL LOL!!

Wow, some comments! stage 3 burner, must be a squid thing. The F-15 has 5 stages of augmentor, not afterburner. Also the run was done inside of a hush house, which is where most high power runs are done, and you can hear the engines running outside. I only ran an engine testing facility at Eglin AFB for 2 years, so I know a little about them.

I was the first person to walk in after this incident, well, me and the Wing Commander. As a matter of fact, the pictures you see on the internet that were taken at NIGHT were taken by me and another QA Inspector. I was working for QA at the time and was involved with the AIB. As was previously stated, the crew chief didn't have the holdback properly attached. As a matter of fact he was told, as he sat in the cockpit, that it wasn't even hooked up prior to the run, and he told the airman on the ground to hook it up. The airman had little experience with this task and asked the engine run crew chief to look at it, but the crew chief said he was sure it was fine. F-15 Engine Troop was accurate with everything with the exception of the number of afterburner stages. While the F100-PW-220 DOES have 5 stages, the F-15E model has F100-PW-229 which has 7 stages.
